Gold Founders Pack / Transfer

Hello,
I purchased the gold founders pack back in 2022 from Glyph and am now told I need to transfer the account. I was able to get the transfer token but when I go to the transfer page here , I am unable to enter anything to recover my account. Any help would be appreciated
